COLLECTION AND PROCESSING OF PRIVATE DATA – PURPOSE OF PROCESSING

Upon your arrival in our premises and before the provision of our
services, we collect the following information of the patient: full name, father’s
name, sex, age, profession, address, dates of visit at the dentistry, the health
complaints and reason of visit, the primary and secondary diagnosis or the
adopted medical treatment, along with any other data of interest, i.e. ΑΜΚΑ
(National Insurance Number), ΑΦΜ (Internal Revenue Service ID), contact
telephone number, which are relevant to the provision of healthcare.
Retaining the aforementioned information, which are updated upon
every visit, constitute legal obligation of the dentist according to Article 12 of
the ΠΔ 39/2009 law, and are archived for ten (10) years commencing on your
last visit. It also constitutes a conventional obligation within our agreement for
provision of dental services.
The process is necessary especially for your identification, the
potentiality of communication, the issuing of tax documents, the needs of
services provision and the monitoring of your health.

RIGHTS OF THE DATA SUBJECT

I kindly inform you that you withhold the right of accessing, correcting,
deleting, restricting, objecting to processing, transferring the data, or filing a
formal complaint at the supervising authority, according to Articles 15-22 of
the General Data Protection Regulation (GDPR).
In order for you to exercise your rights, you can contact us in writing, by

The Hellenic Data Protection Authority (Kifissias Ave. 1-3, Athens) is
the Supervising Authority.

DATA TRANSMISSION

Access to the archive is not permitted to third parties with the exception
of cases which are clearly stated by the existing legislation.
Furthermore, exclusively for the needs of your therapy, you will be sent
text messages on your mobile telephone or via electronic mail aiming at your
updating on your condition or reminding of regular checking. Your personal
data may also be processed by colleagues of the dentistry, including dentists,
diagnostic laboratories, and dental technicians, without your prior special
notification.
Third parties provided with your personal data withhold absolutely no
right to utilise them for any other purposes.
At the same time, dental confidentiality is in force.
